About Your Visit
Financial Policies & Insurance Information
We share your concern regarding the rising cost of health care. We have established financial policies which are necessary to help hold down the overall cost of your care. If you are not covered by insurance, we do require payment at the time of service. We accept check, cash, money order, Visa, Mastercard, American Express, and Discover. If you are unable to pay in full at the time of service, you should make arrangements with the business office before your visit.
If you are covered by insurance, we will be happy to file on your behalf. Please present your insurance information to the front desk personnel as well as any claims forms that we may need. We will collect any copayments due from you at the time of your visit.
Please remember that it is your responsibility to promptly answer any requests for information from your insurance company which might hold up processing of your claim. Normally, you will only receive a bill from us once your insurance company has paid. However, if your insurance company does not pay within 90 days, you will receive a statement from us.
We Accept Medicare Assignment
If you are covered by Medicare, we will file your claim for you as well as any supplement that you may have. Once payment has been received from Medicare and/ or your supplement, you will receive a statement from us for any balance due on your account.
Workers Compensation
We accept workers' compensation claims with proper verification from your employer. We will file your claim with the appropriate carrier. However, if your claim is denied, the balance on your account will be billed to you.
Automobile Accidents & Other Liability Cases
If you are in need of emergency care due to an accident, you will need to bring proof of insurance coverage. If you are in an auto accident and wish to file on your group health insurance, you will need to bring proof that there is no automobile insurance coverage available. If you do not have health insurance and the coverage is liability only, we ask that you pay at the time of services as the liability insurance will reimburse you directly.
Referrals
If you are covered by an insurance plan, such as an HMO, that requires a referral from your primary care doctor, it is your responsibility to make sure that we receive the authorization number before your visit. Not obtaining the proper referrals will result in denial of the claim by your insurance company.
Telephone & Prescriptions
Your are encouraged to call the office regarding your condition, medication, or treatment. Please do not hesitate to contact us with any question you may have concerning your medical care. We would appreciate non-emergency telephone calls being made during regular office hours when your medical records are readily available. Every effort will be made to return your call by the end of the day.
Request for a prescription refills should be made during normal office hours. Please have your pharmacy name, location, phone number, and name of medication available when you call. Refills are called in by the end of the day and you should check with your pharmacy after 5:00pm to see if your prescription is ready. For your protection, pain medication cannot be refilled after hours as we do not have access to your medical records.
Appointments
Office hours are by appointment. We ask that you kindly give 24 hours notice if you must reschedule or cancel your appointment. We will make every effort to insure that your appointment is on time, however the nature of our practice is that emergencies do occur which may cause a delay in the schedule.
We will keep you informed of any delays and notify you ahead of your scheduled appointment time, if possible, to minimize any inconvenience to you.
